Grades K, 1, 3, 5 - A valid Health Exam dated 9/5/23 or newer needs to be on file to fulfill the NYS requirement. In the event that a Health Exam form cannot be submitted, WCSD's District Physician will be available to do so at a date that is convenient for them.

(Hearing, Vision - near/far/color, Scoliosis - 5th Grade girls) not completed by your physician will be scheduled and completed by the school nurse at a later date. Any failing/concerning results will be sent home with a phone call to parents prior.

• Kindergarten - Vaccination records in compliance with NYS regulations need to be submitted prior to the first day of school. We will be working on screening all new students to verify completion as well so we are here to help you if needed and we will reach out if anything is missing.
• 6th Grade - TDap Vaccine is required for all students age 11 and entering 6th grade.
• Students with missing vaccinations will have a 14 day period to show proof of vaccination or provide a valid medical exemption from the vaccination.

(This includes ALL medications - including cough drops, Lactaid, eye drops (artificial tears/antibiotics), certain lotions/creams) - All medications to be given in school require a prescription from your physician and a completed parent permission form.  Students that may be able to self-carry their medication will need to have an attestation form completed by your physician and signed by a parent along with the previously stated forms.  These forms can be found on our website.  All medication should be brought into school by a parent or a trusted adult. No medication should be traveling with your child on the school bus without a self carry permission form.
